TenNor Therapeutics Reports Positive Results for Rifaquizinone in PJI Trial

TenNor Therapeutics has announced positive results from a Phase IIa proof-of-concept (POC) trial for rifaquizinone (TNP-2092), a potential first-in-class, triple-targeting drug candidate administered via intra-articular (IA) injection for treating periprosthetic joint infection (PJI). The findings, which suggest TNP-2092 could cure PJI without surgical intervention, are a significant step forward in addressing this challenging condition.

Periprosthetic joint infections (PJI) are a devastating complication that can turn a successful joint replacement into a medical crisis. In the United States, approximately 45,000 new cases occur annually, while in China, the number stands at 22,000. Conventional treatments, often involving surgical debridement and antibiotic therapy, carry high failure rates, especially for chronic infections, and can be both painful and costly.

TNP-2092, developed by TenNor Therapeutics, is a novel biofilm-eradicating agent designed to address the critical unmet need in implant-associated biofilm infections. The drug candidate works by synergistically inhibiting bacterial RNA polymerase, DNA gyrase, and topoisomerase IV, conferring potent activity against resistant pathogens such as Methicillin-resistant Staphylococcus aureus (MRSA) and Quinolone-resistant S. aureus (QRSA). Clinical trials have shown that TNP-2092 outperforms current standard-of-care antibiotics in both in vitro and in vivo models.

The Phase IIa POC trial, conducted in China, evaluated IA administration of TNP-2092 in conjunction with systemic antibiotic therapy in patients with early or acute PJIs. The trial also included patients with chronic PJIs, requiring long-term antibiotic suppression therapy. As of July 1, 2026, the data lock point (DLP) for interim analysis, 10 patients were enrolled. Among the seven patients with culture- or next-generation sequencing (NGS)-confirmed Gram-positive infections, all patients with early or acute PJI who received TNP-2092 IA had completed the 6-month follow-up. The results showed a 100% treatment success rate (5/5). Similarly, among the four patients who completed the 12-month follow-up, all had a 100% treatment success rate (4/4). For the two patients with chronic PJI, 50% (1/2) achieved treatment success at the 6-month follow-up, with 12-month follow-up data pending.

Safety data from the trial were also positive, with no treatment-emergent adverse events (TEAEs) or serious adverse events (SAEs) reported. Nine patients were included in the safety analysis, and the remaining patient, still undergoing follow-up, reported no TEAEs or SAEs as of the DLP.

These results have led TenNor Therapeutics to initiate a multicenter Phase IIb trial. The trial has received approval from China's National Medical Products Administration (NMPA) and aims to further evaluate the efficacy and safety of TNP-2092 IA. The company hopes to bring this potential life-changing therapy closer to patients, offering a non-surgical approach that could fundamentally transform patient outcomes.

"The data from this trial are extremely encouraging and represent a significant step forward in treating periprosthetic joint infections," said Professor Li Cao, Chief Physician at the Orthopedics Center of the First Affiliated Hospital of Xinjiang Medical University and Principal Investigator for the POC trial. "TNP-2092 has the potential to cure PJI without subjecting patients to the painful and prolonged surgical pathway. This is a paradigm shift in PJI therapy."

Professor Javad Parvizi, MD, FRCS, Professor of Orthopedic Surgery at Acibadem University and Co-Chair of the International Consensus Meeting (ICM) on PJI, expressed similar enthusiasm. "Orthopedic surgery has seen remarkable breakthroughs, but the management of PJI has remained stagnant. TNP-2092 represents a truly transformative innovation that could significantly improve outcomes and quality of life for PJI patients."

With these promising results, TenNor Therapeutics continues to advance its efforts to address the critical unmet medical need in implant-associated bacterial infections. The company's pipeline includes Rifaquizinone (TNP-2209) as a triple-targeting antibacterial drug candidate for treating various implant-associated infections, including periprosthetic joint infection, left ventricular assist device infection, and catheter-related bloodstream infection.

The ongoing multicenter Phase IIb trial will provide further insight into the potential of TNP-2092, with the aim of delivering a breakthrough therapy that could revolutionize the treatment of periprosthetic joint infections.
