Study Links Chronic Traumatic Brain Injury Symptoms to Irritability

A new study from the University of Colorado Anschutz Marcus Institute for Brain Health reveals that irritability experienced by military veterans with chronic mild traumatic brain injury (mTBI) is linked to a combination of psychological and sensory factors, rather than a single cause. The research, published in the Journal of Neuropsychiatry and Clinical Neurosciences, suggests that irritability is strongly associated with post-traumatic stress disorder (PTSD) symptoms, chronic pain, and difficulty understanding speech in noisy environments.
The study, led by Jeffrey Hebert, PhD, PT, associate professor at the CU Anschutz School of Medicine, involved 124 military veterans, 61 of whom had a history of chronic mTBI, and 63 who did not. Participants completed standardized assessments that measured irritability, PTSD symptoms, chronic pain, balance, and auditory processing, including speech in noise comprehension.
Veterans with chronic mTBI reported significantly higher levels of irritability, more severe PTSD symptoms, greater chronic pain, worse balance, and increased difficulty understanding speech in noisy settings compared to those without a history of brain injury. The findings indicate that PTSD symptoms had the strongest association with irritability, followed by chronic pain. Difficulty understanding speech in challenging listening environments also contributed independently, suggesting that sensory strain plays a role in emotional regulation.
The study highlights the complexity of long-term symptoms after concussion, particularly in military veterans. It emphasizes that irritability is often viewed as a purely psychological issue, but the research suggests it may instead reflect changes in brain function, sensory processing, and emotional regulation. This insight supports the need for more comprehensive approaches to care, including addressing PTSD symptoms, chronic pain, and sensory processing difficulties together.
The results also have clinical implications, indicating that clinicians treating patients with chronic mTBI may improve outcomes by screening for PTSD symptoms, chronic pain, and difficulty understanding speech in noisy environments when irritability is present. Future research will explore whether improving functional hearing and sensory processing could reduce irritability and enhance quality of life after concussion.
The study underscores the need for a more integrated approach to managing the long-term effects of mTBI, particularly in military veterans. It highlights the importance of addressing multiple factors to effectively support those affected by these conditions.
