Sentara Health, a leading healthcare provider, has chosen GW RhythmX, an AI-powered precision care company, to advance the future of primary care. The pilot initiative aims to explore how real-time, patient-specific clinical intelligence can be integrated into clinician workflows, enhancing personalized, proactive, and coordinated care.

GW RhythmX's Precision Care Platform analyzes patient history, evidence-based guidelines, care gaps, social and financial determinants, payer requirements, formulary considerations, and other relevant clinical data. This technology surfaces personalized recommendations during patient encounters, providing actionable intelligence at the point of care. The comprehensive data within the RhythmX AI platform also enables more precise navigation and orchestration of care delivery, leading to improved access.

"Health systems are redesigning care models to improve access, support increasingly complex patient populations, and empower clinicians to practice at the top of their licenses. The challenge is ensuring these models can be consistently executed across every patient encounter. We believe AI-powered precision care can help bridge the gap between strategy and execution by delivering the right intelligence to the right clinician at the right moment."

The pilot initiative builds on GW RhythmX's growing momentum as health systems seek solutions that support real-time clinical decision-making. Following the launch of RhythmX Pulse within Microsoft Marketplace and Dragon Copilot, GW RhythmX continues to work with healthcare organizations pursuing more personalized and scalable models of care delivery.

Prasanna Mohanty, EVP & President of Ambulatory Care at Sentara Health, expressed enthusiasm for the partnership. "Health systems are actively exploring innovative approaches to care delivery that can improve access, efficiency, and patient outcomes. Technology has an important role to play in supporting clinicians and care teams as these models evolve. We look forward to evaluating how AI-powered precision care can help bring relevant patient information and clinical intelligence into workflows at the point of care."

GW RhythmX, formed by the merger of Get Well and RhythmX AI, pairs its clinical recommendation technology with established patient engagement infrastructure. The company's solutions are deployed across more than 150 health systems, reaching over 85 million patients, including 8 million U.S. military veterans. GW RhythmX's solutions were recognized in 2024 by KLAS Research, Fierce Healthcare, and AVIA Marketplace. The company is a subsidiary of SymphonyAI Group, drawing on more than $1 billion in R&D investment and longitudinal data spanning 300 million patients, 4.4 billion annual claims, and 1.8 million healthcare professionals across more than 3,000 facilities globally.

Sentara Health's selection of GW RhythmX for this pilot initiative underscores the growing importance of AI in transforming primary care. The partnership aims to operationalize emerging care delivery models, leveraging AI to enhance patient care and streamline clinical workflows.
