Nuna Launches Free Chronic Care Coaching Program for Medicare Beneficiaries

SAN FRANCISCO, Sept. 15, 2026 /CourierPR/ -- Nuna Health, a digital health company, has launched a no-cost digital chronic care coaching program for eligible Medicare beneficiaries in markets where the company operates. This initiative is part of the CMS Innovation Center’s ACCESS Model, which aims to tie payments to the health outcomes achieved by participating organizations.
Nuna’s AI-driven platform pairs a 24/7 conversational health coach with a personalized care plan, education, and gamification to help patients manage their chronic conditions. The program is designed to support patients in building consistent, sustainable habits that can be maintained between clinic visits and everyday life at home.
Dr. David Ansell, a primary care physician and Nuna development partner, highlighted the impact of the program. "As a primary care physician, I've struggled at times to get patients to care about their blood pressure. Nuna changed that. It gives patients a simple tool to build healthy habits at home. I've seen it help patients stay engaged with their blood pressure between visits," Ansell said.

Nuna's platform is available for download on the Apple App Store and Google Play. The company is supporting beneficiaries enrolled in the Model's Early Cardio-Kidney-Metabolic (eCKM) and Cardio-Kidney-Metabolic (CKM) tracks, which cover conditions including high blood pressure, high cholesterol, prediabetes, type 2 diabetes, chronic kidney disease, and atherosclerotic cardiovascular disease. Eligibility is determined by CMS based on a beneficiary's specific conditions and other criteria.
The program is part of a broader effort to improve chronic care management for Medicare beneficiaries, leveraging technology to bridge the gap between clinical visits and daily health management. By providing continuous support and education, Nuna aims to help patients achieve better health outcomes and reduce the overall cost of care.
