FRAMINGHAM, MA, UNITED STATES, October 8, 2026 /CourierPR/ -- Medicare patients in Massachusetts now have a new support system to help them manage chronic conditions between doctor visits, thanks to a program offered by MDinteractive Health. This initiative, part of Medicare's ACCESS Model, provides patients with regular check-ins and support at no cost, aiming to make daily management of chronic conditions less daunting.

MDinteractive Health, a participant in Medicare's ACCESS Model, has launched a program that offers Original Medicare patients in Massachusetts steady, personal support between visits. The program is designed to help patients with chronic conditions such as high blood pressure, high cholesterol, prediabetes, diabetes, kidney disease, heart disease, chronic pain, or depression or anxiety. Patients can call (888) 634-0815 or visit mdinteractive.health to check their eligibility.

At the heart of the program is Sarah, MDinteractive Health’s AI care assistant. She provides regular check-ins by phone between appointments, helping patients stay on track with their medications and readings. Sarah also shares guidance for their condition and keeps the patient's own doctor informed. For those whose care plan includes blood pressure monitoring, MDinteractive Health provides an easy-to-use home monitor at no cost. The monitor sends readings to the care team automatically, ensuring that patients can simply put on the cuff and go about their day.

According to Paulo Andre, MD, founder and Medical Director of MDinteractive Health, the program aims to provide steady support during the critical periods between doctor visits.

MDinteractive Health has been a CMS Qualified Registry since 2010 and is ONC Certified Health IT. The company is located at 945 Concord St., Framingham, MA 01701. Patients can learn more about the program and how to participate by visiting mdinteractive.health.

The program is voluntary and does not change patients' Medicare coverage or benefits. Patients continue to see their own doctors and keep every one of their existing medical plans. MDinteractive Health is committed to providing no-cost, between-visit support to help patients manage their chronic conditions more effectively.
