Family Practice Center (FPC) and Sylvan Health have partnered to expand access to personalized nutrition care across Central Pennsylvania, bringing evidence-based support to over 200 providers and their patients. The collaboration, now available across 41 FPC locations in 12 counties, aims to integrate nutrition support into routine primary care, addressing both chronic conditions and lifestyle goals.

FPC, a physician-owned primary care organization serving Central Pennsylvania, will extend the reach of its services through the addition of registered dietitians provided by Sylvan Health. This integration allows FPC to offer personalized nutrition care as an extension of its existing care teams, ensuring that patients receive comprehensive support for their health and wellness.

According to Benjamin Williard, CEO of Family Practice Center, "At FPC, we believe the best healthcare doesn't just treat illness; it helps prevent it. By expanding access to nutrition support, we can give our patients practical, sustainable tools to build healthier lifestyles, manage chronic conditions, and maintain their quality of life as they age."

Sylvan Health's model integrates registered dietitians into existing clinical workflows, providing a seamless and efficient way for providers to offer nutrition care. The program supports referrals, scheduling, documentation, and billing, making it easier for both providers and patients to manage the care process.

Early results are promising, with 95% of patients reporting satisfaction with their nutrition care and 97% returning for follow-up visits. These metrics highlight the value that personalized nutrition support brings to patient care and underscore the effectiveness of the partnership.

Rhodes Amaker, CEO and Co-Founder of Sylvan Health, noted, "Primary care providers are on the front lines of helping patients prevent and manage chronic disease, but they don't always have the time or resources to provide the nutrition support that can make a meaningful difference. Our work with Family Practice Center demonstrates what's possible when nutrition becomes a seamless extension of the care team, giving patients accessible, personalized support while making it easier for providers to deliver more comprehensive care."

Family Practice Center serves a diverse patient population, ranging from newborns to older adults, with a commitment to delivering high-quality care that meets the needs of the communities it serves. The collaboration with Sylvan Health reflects the broader mission of making personalized, evidence-based nutrition care a standard part of healthcare.

Through this partnership, Family Practice Center and Sylvan Health aim to enhance the overall health and well-being of patients across Central Pennsylvania, providing them with the tools and support needed to lead healthier lifestyles and manage chronic conditions effectively.
